* All ProtoComm entries are explicitly enabled by default inside
  ProtoComm::start()

* MfeaVif::start_protocol() would not start the MfeaVif if it was
  not running before. Not sure it is the right thing to do.
  The alternative is to require the MFEA client to send start_vif() XRL.
